  WHO IS BENEFITY ŠKODA

  CAFATERIA INTENDED FOR?

  It is intended for employees who have

  used Health Benefi ts and worked

  atŠKODA AUTO for min. 3 calendar

  months. Now even for exempted

  employees or employees on trips

  ormaternity leave.

  An Internet application where you can buy just like in any other e-shop

  and pay with the points you have collected or where you pay for products

  and services at contractual partners.

  Benefi ty ŠKODA cafeteria for new employees

  New employees can start collect points from the Day 1 at work if they meet

  all necessary conditions. They will see these points in @SMART App Benefi ty

  ŠKODA – points in ŠKODA Space.

  Points will be transferred to cafeteria automatically after you have worked

  with us for 3 calendar months. Your cafeteria account is opened on the same

  day, and you can start using your points.

  They will be automatically entitled for Flexi Pass CARD in the 3

  rd

  month

  of their employment. Instructions on how to get the card are included

  inpayroll. In Mladá Boleslav, the card can be collected in a hostel between

  the Administrative Centre and Petagon, Room No. 010. Collect it at HR Dpt.

  inKvasiny and Vrchlabí.

  Employees who are transferred from an agency and are a part of the “Master

  Vendor” system, will have their account opened on the Day 1 of their work.

  They can collect the card at distribution location from the 15

  Employee Profi le

  In settings below your profi le, you can enter your own

  E-mail – if you wish to get e-mail notifi cations on points credited to you,

  your use of points, orders, or expiration dates.

  Phone Number – can be set in the upper corner in the settings menu.

  It is used as a payment possibility at contractual partners who are not

  equipped with a payment terminal.

  You can request that a balance of your account be sent to the number

  you provided. Please send an SMS to number 773 78 79 79 and use the

  following text: CAF space INFO space KONTO. The SMS is charged in

  accordance to your current tari .

  due date of your points’ validity in

  the cafeteria, in ŠKODA Space and

  also in your payroll. Do you wish to

  receive e-mail notifi cations?

  Points that you acquire in such a way are valid until

  December 31, of the calendar year ensuing the day of the

  points’ acquisition.

  All points from Health Benefi ts that have not been used were

  transferred to the B e n e fi t y Š K O D A cafeteria.

  These transferred points are valid until December 31, 2020.

  Health benefi ts application was terminated on the day

  Benefi ty ŠKODA cafeteria was launched.

  ONLINE PAYMENT GATES

  Click on the Payment Gates banner in the cafeteria. Here you can

  fi nd a list of e-shops where you can make purchases, and select

  employee benefi ts – Sodexo as a payment method when paying.

  KARTA FLEXI PASS CARD

  You can pay with the card at all contractual partners who accept

  it. These partners are marked by the Sodexo logo and fall into the

  health or sports categories.

  A simple way to pay at vendors without payment terminal. For this

  Flexi Pass CARD is a contactless benefi ts card that works in the same way as regular

  card issued by a bank. It is connected to a Benefi ty ŠKODA cafeteria account, where

  you can fi nd out your current balance of points and history of use, among others.

  Manage your card directly in cafeteria, have it blocked here, request issue of a new one,

  and reset your PIN. Cafeteria is described on pages 8–10.

  When paying for the fi rst time, insert

  the card into the payment terminal.

  Your card has been activated

  and you can start paying with it.

  From then on, the card is contactless. You enter PIN only when paying over CZK 500,–.

  Flexi Pass CARD is a contactless benefi ts card that works in the same way as regular

  card issued by a bank. It is connected to a Benefi ty ŠKODA cafeteria account, where

  you can fi nd out your current balance of points and history of use, among others.

  Why did I get a new card was it impossible to use the MFA?

  MFA would not be possible to use in the network of Sodexo partners throughout the

  Czech Republic. In order to pay with a card, it is necessary to have a payment terminal

  with a card reader.

  I did not fi nd any pharmacy or any other business where I would like

  tomake use of my points. What shall I do?

  Use the cafeteria to request inclusion of a new contractual partner – Partner

  Suggestions window (see page 9) and fi ll in necessary data of the partner suggested.

  Thepartner must meet conditions arising from applicable law on income tax for making

  use of benefi ts, and be interested in becoming Sodexo’s contractual partner.

  Will other benefi ts remain valid?

  Yes, any other benefi ts as confi rmed in the Collective Agreement shall remain valid

  Will other benefi ts be included in the cafeteria?

  Based on possibilities available, the o er of benefi ts will be extended with cultural

  events, books etc.

  Why do points expire and are not transferred automatically to additional

  pension scheme savings?

  ŠKODA AUTO wants to motivate its employees to have an active approach.

  Will I receive an e-mail notifi cation about the change of amount of points?

  Information on points acquired, ordering of benefi ts, expiration date of points due soon

  (see page 11) can be received by an e-mail, if you set this in the cafeteria.

  Is it possible to search for particular products in the cafeteria – e.g. nasal

  drops?

  You can search for shops and their locations, not particular products.

  Can I lend my card to somebody else?

  The card should not be lent to anybody else, as the card is intended for the particular

  employee, and is issued to his/her name and personal number. A contractual partner is

  entitled to confi rm identity of an employee who uses the card.

  Can I load my card in the mobile app?

  Not for the time being. Application for club cards cannot be used as it is a payment card.

  Will points be added in the cafeteria as a reward for ZEBRA improvements?

  From taxation reasons, points collected in the ZEBRA app cannot be transferred

  andmade use of in the cafeteria.

  Can I load my own money into the cafeteria?

  It is not possible to load own resources into the card.

  How do the points from Health Benefi ts get transferred to the cafeteria?

  Points are transferred automatically to the cafeteria.

  How do the points from the cafeteria get to the card?

  A card is linked to a cafeteria account, so there is no need for you to transfer your points.

  A balance on your card is equal to that in the cafeteria.

  What is the validity of the card and what is the process of its replacement?

  The card is valid for 3 years, after which you will be issued a new one automatically.

  A replacement for a lost card must be ordered in the cafeteria by ourselves. It will

  bedelivered to a distribution location.

  Is the cafeteria intended for regular sta of ŠKODA AUTO only, or is it

  available to agency employees, too?

  In the 1st stage, the cafeteria is open for regular sta of ŠKODA AUTO only.

  Will the possibility to make use of points in pension scheme savings

  or for purchases at Aramark and in the cafeteria be maintained?

  Yes, employees themselves can decide on how will they use their points.

  Upon transfer of points to Aramark, employees take goods marked as “healthy food”

  inrestaurants and kiosks. They will pay at the cash desk with their MFA card. At the

  endof the month, the system shall assess if employees have any points transferred

  totheir account, and subsequently pay less from their salaries.

  10% of healthy food is paid from salaries of employees due to the fact that we want

  todecrease taxation incidence to employees. 90% of the price shall be deducted from

  their points transferred.

  You can ask for your points being transferred to additional pension scheme savings

  until the 20th day of a month. Points ordered after this date shall be transferred in the

  following accounting period.

  Is it possible to fi nd out about the amount of my points anywhere else than

  in the cafeteria?

  Detailed information on the amount of your points and changes is displayed in the

  cafeteria. Points credited to your account are displayed in @SMART App – Benefi ty

  ŠKODA – points.

  What is the validity period of points I acquire?

  Acquired points are valid until December 31 of the calendar year ensuing the day

  of the points’ acquisition and their display in @SMART App – Benefi ty ŠKODA –

  points. Employees will be notifi ed of the end of points’ validity in the cafeteria and by

  astatement in their payrolls. A notifi cation e-mail will be sent after entering your e-mail

  address in the cafeteria.

  What is the value of a point?

  1 point has a value of CZK 1,–.

  Can I make use of any amount in the cafeteria?

  Yes, any random amount rounded o to a full point up to the balance of your account.

  If validity of my points expires and I have some remaining on my account,

  will I lose them?

  Yes, any points that have not been used will expire.

  What healthy food is included in what I can make use of in the cafeteria?

  A complete list is located in Aramark section in the cafeteria.

  Can I get a summary of my purchases of healthy food?

  Yes. You can get the summary in @SMART App – Benefi ty ŠKODA – points in the

  Balance of your points tab in Aramark.

  Will my points expire when I terminate my employment at ŠKODA AUTO?

  No. In the case of employment termination, your points will be discharged in your salary.

  What shall I do if I am at a loss with the cafeteria, or if I fi nd discrepancies

  on my account?
